AIM:
To demonstrate a wheelchair to bed transfer to a person with complete T4 tetraplegia.
Rationale:
The therapist demonstrates the key components of the task to the patient
Equipment:
- Height-adjustable wide bed
- Wheelchair
Key Points:
The sub-tasks (in order) are:
- Positioning the wheelchair at 45 degrees to the bed
- Putting the brakes on
- Moving to the front edge of the wheelchair
- Positioning the feet on the floor (not shown in video clip)
- Positioning the hands
- Lifting and shifting the body onto the bed
- Lifting the legs onto the bed
Common errors:
- Positioning the wheelchair – bed too high, ie, higher than the top of the wheelchair cushion
- Moving to the front edge – failing to initially turn the front casters forwards. This error will result in the chair tipping forwards when the patient moves forwards
- Positioning the feet – dropping the feet and scraping the heels along the footplate
- Positioning the hands – falling forwards when attempting to position the hands
- Lifting and shifting – failing to adequately lift and move forwards, thereby scraping the buttocks across the wheel
- Lifting and shifting – failing to rotate about the shoulders when lifting (ie. head down and buttocks up), thereby failing to get sufficient clearance under the buttocks
- Lifting the legs – failing to ensure the first lifted leg does not fall back onto the floor when attempting to lift the second leg
